child.
We encourage you to make an appointment
to visit the school during a school day.
Shadowing days are available for transfer students who want to
experience learning in the Our Lady of Malibu
School Community.
APPLICATION
PROCESS STEPS
- Complete two page application form for each
child. ***Applications
are available at the school office or online.
- Return the application to the school with the
following items:
- Copy of Birth Certificate
- Copy of Baptismal Certificate (Catholic
Students)
- Copy of immunization records
- Copy of most recent report card
- Signed permission form to seek recommendations
from previous school(s)
- Attach a photo of your child to the application
- After all of the paperwork has been received by
the office, you will be contacted for a date for:
- Testing and interview
- After acceptance, forms for registration and
selection of fees will take place at the school office.
